Simply put, if nobody dies and the healer doesn't have to drink after every normal pull, he's doing a good job.
Hmm. I guess that makes both of us pretty good healers. Personally I've noticed that in instances I spend only up to 5 bottles of water per hour, usually less (like 1-2), and people very rarely die because I didn't do my job.

From the outside looking in (gasp!), I'd have to say the good healer is the one that knows what the other classes are actually doing, and can thusly prioritize. Maybe this is just a component of being a good group member, but that would seem one of the more important parts to me.
Say, if we come to an AoE section, then the Mage (coughme) should naturally take priority, else I'm going to either a) be squashed, or b) be ripped into itty bits and pieces. A shield is nice for being prepared, and so is an HoT.
Whereas, we come to a boss encounter, I hope you're keeping the tank up and not me. I really don't need a Renew: prolly just a downranked Flash Heal every now and then, or its equivalent.

a good healer needs a very good understanding of ALL his/her spells and how they effect the class's he will be healing,
also a mb macro and maybe a aggro macro for added ease, put all this together with a averagely skilled played with average experience of wow and you will have the groups efficiency greatly improved :)
anyway, things i see that i think make good healers are, keep HoT's topped up on the tank, simple, 2nd use power word shield on cloth & leather wearers the second they pull threat followed by a regen, it gives them confidence that the healer has ther back =) use faint as much as possible, it owns :) i only use big heals once ppl get below 30% and then HoT and see wat unfolds, one last thing is that the size of a pull should be based on the mana of the healer, the only exeption is MAYBE if a warrior has loads of rage and im nearly full on mana, ya know the whole efficiency thing lol
i also agree with the whole, certain class's take priority at certain parts of instances and therfore the healer needs to adapt, but basically a skilled player that plays logically with a good knowlage of the spells will pwn till he's outta water :D

Rules of thumb:
Knowing what is going to happen before it actually happens. Start your heals before dmg is taken.
Know your group. How fast do they adapt. How much damage will the other players take.
Know your spells. Heal as much as you can for minimum Mana. Try to place your spells in a way that may put you outside the 5-second-rule if you can.
Don't panic. ;-)

To boil it down to a few bullet points (sigh... have worked in an office for too long), it comes down to this:
- keeping people alive (reflexes, awareness, knowing your abilities)
- not drawing aggro (timing, threat-awareness, talents, knowing your abilities)
- not running out of mana (gear, knowing your abilities)
- not being a complete noob (using fear, walking into adds, etc)
Exactly how you go about achieving those things depends on your exact class as well... some things - like threat reducing talents and a threat-metering add-on and decent gear - will help with specific issues, but the main thing is knowing your class abilities and using them well and in time ;)

- Knowing your role and how to maximize the effectiveness of your role through talents, gear and consumables.
- Knowing the role of others in your group and how to maximize your synergy with them.
- Knowing and preparing for each encounter in advance.
- Being aware of your surroundings at all times. I've seen many healers that die to void zones or cave ins because they were too busy staring at bars and not at their surroundings.
- If healing is covered, add some DPS to the mix as well. A priest that is twiddling his/her thumbs when people are taking little to no damage is not maximizing his/her efficiency. Throw up some DoTs and/or wand the mob.

Healers that can judge their heal ability compared to the amount of health needed by group mates and also judging who to heal first based on aggro lvls and damage taken. Basically ^^^ said up there a great healer doesn't let anyone die, take aggro, overheal, or waste mana. People can die in groups no matter how great you are, but how you handle it says a lot about your ability to play your particular race/class combo.
