A priest in my guild does... I guess he just keeps his heal target targeted and has spells on his actionbars... Personally, I use grid/clique and don't see how someone could get by with just the build-in without neglecting raid heals. My healer is a pally, so perhaps it's simply more important for me to be able to easily cast on the raid, for I can simply beacon the MT. The priest I mentioned has always been a bit of an anti-addon guy however.
There are so many options for good healing setups via addons that I see no reason not to use one.

You can use clique with the default UI but that is neither here nor there.
I never learned to bring up the default raid frames until way after I'd been using other frame mods. The best I could tell was that it would be harder to track buffs but still doable. I found the health bar to be too narrow and too long for my tastes. Understand though I only used it once or twice and it was a long time ago. Someone tried to convince me that my raid frames where slowing me down because the memory it was using. In the end I found that you can't heal what you can't see and as I couldn't see what the heck was going on I went back to a more modified UI.
As for Vlet's priest friend. There is more than one way to heal than grid/clique. The way your priest friend is probably it is by hotkeying his spells. Click the person you want to heal then hit the hotkey. You can switch to the next target while your heal is casting to save time. If he really knows macros then he can generate mouse over ones so that all he has to do is put his mouse over the player and hit the hotkey. Once you learn how to heal this way your just as fast as any grid/clique healer even for raid healing.

I can't picture the default raid frames off the top of my head, but I'm pretty sure the curse/disease/poison effect isn't very noticeable, which would make it difficult to know who needed cleansing.
For me to use the default raid frame, I'd have to use Clique at the very least. I began healing 5mans with the default UI, but found the 2-step-per-heal (select, apply) a bit cumbersome. Clique makes it a lot easier.

Pretty sure debuffs are just another aura icon amongst the others present.
At least thats what i gather when i see a really long reaction times to posion/curse mages and paladins i know are using default UI.
"now which one of these icons means this person is cursed...<mouseover over the first one>...no, no this isnt it...maybe the next one."
Meanwhile the raid has blown up.
