First off, the goal here is to not have the healer ever pull agro. If a healer has agro, that means you haven't hit it at all; you should use Thunder Clap early to make sure you tag everything.
There isn't a macro or anything that will automatically pick that target. I always play with Mob Name Plates on, so I have a bar I can click on to switch targets quickly.
But the first thing you should work on is never allowing a healer to have agro. It just shouldn't happen.

If healers are overagroing you arent doing your job correctly.
Make sure you tank facing your group, this way you can easily see what goes after ranged dps and target/taunt it.

When tanking, and the healer pulls aggro
you've got bigger problems if this happens. healing threat is so low now, it really has to be an untouched mob to target a healer. make sure you're picking up everything you're supposed to.

When this happens to me, after looking at recount I usually find that the tank is not using Cleave at all. Make sure you're Cleaving in addition to using Thunderclap. Glyph of Cleave is win for when you're still gearing your tank through heroics.

While cleaving really isn't required for tanking heroics, it's crazy how good it is. I used to just kind of manually queue cleaves, but I've seen macroed it in with my Revenge (replacing my old HS macroed with Revenge) and it's pretty amazing. Every 1.5 seconds having an extra like 1500-2000 damage pop up on your screen, it's really nice.

i'm using grid, which shows a red border on the one who pulls aggro. but as stated before, you should never ever have a healer pull aggro. if that happens you're doing something wrong^^.
just use omen and always have an eye on it. press "v" so you can see the mob's healthbars.
grid showing aggro is incredible useful for my disci. -> if someone pulls aggro just shield him and be prepared to heal him even more

Hmm. While it's not a set in stone rule (THOU SHALT CLEAVE THINE ENEMIES), and if you're very good at warrior tanking you can probably do just fine without using it... well, every bad warrior tank I've healed, after looking at the recount damage breakdown...Cleave's not even on the list. Whereas those that do, it's up there, usually 3rd or 4th on damage dealt.
TLDR: When tanking any kinda trash, I pretend to be Saurfang jr: I cleave while I cleave, so I can cleave while cleaving.

In most cases yes, but what about encounters where mobs stagger themselves in a way that make Taunt/Intercept necessary?

when I tank heroics usualy my top dps ability is cleave (glyphed)... its warriors only spamable multitarget ability... use cleave every weapon swing and thunder and shockwave every cooldown. (idealy tab between targets when you tank more then 3 mobs)
about targeting, you can set up healbot to target players target (but the one who got aggro must be targeting that mob)...
or use clique and set intervene to be used on click, works great.

In the case of a healer getting aggro, the target will 99% of the time be another party member, right? How best to determine which mob doesn't like the Healer other than visually and clicking the offending mob chevron?

Who gets aggro depends on a lot of things, aggro has a lot of weird little mechanics behind it that as a tank you have to learn to recognize and adjust to quickly. Watching meters and trying to see which mob likes the healer most is going to lead to missed rotations and mistakes, which will then dump aggro onto dps or heals. Your best bet is to just plain make sure aggro is stuck to you like glue.
Thunderclap, cleave, switch targets, cleave again. If it's a large trash pull and you have strong AoE dps charge the skull, thunderclap, cleave, shield slam on the skull, then switch targets and keep a cleave going through the group. Almost all pulls in all instances are about the same really, just the mobs look different and may have a slightly different characteristic or two between them. But aggro management on packs is almost exactly the same no matter where you're at, once you get the feel of how to handle them it's incredibly easy, and requires no meter watching.
In today's Wow, where most all dps have very strong burst AoE potential, a warrior tank must learn to effectively switch targets and keep threat up at all costs. Which is why I'm such a big fan of cleave; it's a warrior's only spammable multi-target threat component. Use your tools, understand how they work and why, and all the aggro is belong to you.

I have to say, the only trash pack i have massive troubles with is in the Pit of Saron. That group up the hill's caused 7 wipes until i finally got it right. They are just so dam hard to control as a warrior, whereas a paladin has it easy mode with a silence.
Cleave is very good. i usually put omen close to my eye line when AoE tanking, tabbing through to see the threat of each mob, you will find that some DPS just target one mob and go for the burn, if this happens i can pick it up in omen, shield slam, put 4 stacks of sunder up pretty quickly and continue to tab cleave knowing i just put down 40k worth of threat.
Shockwave stun is actually been amazing helpful. Other packs can be shockwaved while still in the middle of your 1st pack without having to target them. very good emergency stop button while you gain control and placement.
However, above all else, what i have found to be of massive help is Block. Getting a SBV set up is amazingly helpful for aoe threat gen.
The only time a healer got aggro from me is when he decided that it's imba to heal with Righteous Fury on.

your taunting your healer. only way to do it is to change the macro to target your targets target, and hope your healer is targetsing the mob (which they won't be).

Another thing that would help, and hasn't been mentioned, is knowing the fights. If you know an add is going to spawn, and where, then picking them up is easy.
I've tank stuff and the first time is always terrible. So bad that I feel sorry for the dps. But the next time is so much better, it's night and day. I know the pulls. I know the mobs. I know where they're coming from, when they'll spawn, etc.
Some mobs even have a de-aggro function to them. Learn who and when.
Learn each pull.
Also, some mobs are designed to target others (like a healer) and blast them with something. So it may not be your fault, you just may need to change the kill order and kill the problematic mob first.
