Use
IT > PS > SS > BSx2 > DC Repeat. For single target
IT> PS > Pest > DnD > IT > PS > Pest > BS > SS
Glyphs are fine, You're over hit cap, spec is fine, Your gear is slightly better for Blood with all the Armour Pen., Haste is a better cloak enchant for all specs but especially Unholy.

With all that armor pen you should either be blood or dw frost which depends on armor pen. With unholy you need more crit. Armor pen is nice, but not necessary. If you want to stay unholy change the glyph of icy touch to plague strike for the increased damage. The icy touch glyph is changing for the icecrown patch, but it hasnt changed yet and i find that I always have enough runic power for Death coils.

BS comes before SS to trigger Desolation to give your SS 12% increased damage.
Drop 5 Expertise: you want 26, not 31.
You can lose almost 90 Hit, so if you have some pieces you can swap in, do it. You want to sit at 263 Hit.
Move the Nightmare Tear from your chest into your helm to meet that 8 Strength socket bonus.
Enchant your bracers (50 AP), gloves (Crusher), boots (Tuskarr's Vitality) and helm (Arcanum of Torment from Ebon Blade Revered).
And yeah, you have a lot of Armor Pen. Crit and Haste are both better than ArP in the current iteration of Unholy. Next patch changes that somewhat, so hang onto the ArP gear.
Outside of all of this, your biggest problem is probably your gameplay. This is not an RNG spec: you're not waiting on procs. You're making the damage, not the game.
That being said, make certain you _never_ let your diseases fall under any circumstances outside of a target switch or a fear or whatever. If you're on-target, it should be diseased.
You need to use Gargoyle at the correct time as well.
