This will get a little long winded Entzo,
I have my numpad set up for all my action keys. With slot 11 as . and slot 12 as +. I grafted bottom left as CTRL plus the numpad keys. So 24 keys right there.
To reduce use of my mouse I put my strafes on A and D and put my turns on num4 and num6. (The abilities associated with num4 and num6 are now alt-num4 alt-num6 battle shout and bloodrage respectively.)
Then I made a stance dancing macro that I assigned to the E key. Finally I put Sunder Armor on F for quick rage dumping.

souldier,
Does that mean when you PvP it's almost just like home rowing your way through your opponents? Just out of curiosity what's your reply key? (I noticed you changed R).

You asked about the default movement keys...I use ASDF instead of WADS to move about.
A - backpedal
S - strafe left
D - strafe right
F - forward
Using the right-mouse buttons for camera control during combat means you don't need to turn with the keyboard & I simply deleted those bindings altogether.
I use the standard
123456
keys for the main toolbar, but instead of the clumsy
7890-=
icon slots I use
qwert, g,
and
cvb
. Holding down
CTRL
with
123456
gives another half-toolbar. To circumvent the inability to press certain keys while moving I use a combination of strafing & camera turning. To prevent these odd situations as much as possible however, I often place abilities which require you stop moving (such as Pyroblast, Aimed Shot, etc.) on the
CTRL- keys. The goal is to keep as many instant common-use abilities accessible as possible without clicking or holding a modifier button down. For trinkets & buffs I use CTRL
+
ALT
modifiers on a right-side toolbar w/ non-combat spells filling the remainder of the slots.
In addition to the main toolbar I use Mouse 4 & Mouse 5 to flip toolbar pages up or down; one bar up or down translates to an additional 24 keys (often used for low-rank spells or non-priority instants, professions & tracking). I use
ALT
+
123456
for class specials, such as Stances, Shapeshifts, Auras, etc.
Without add-ons, I have 14 slots with 1-finger access to instant abilities...with a page flip up or down via the mouse I gain an additional 12 (per bar).
The final total for all buttons on screen at any one time is 48, plus an additional 24 accessible via page flips & up to 6 class-specific actions.

It should be worth noting that I use a Microsoft Natural Elite "split-key" ergonomic keyboard. For those who do not know what this is, there are pictures here:

As you can see by the image, my keys are limited horizontally to Q through T, A through G & Z through B put plainly.
It's a lot easier to type than any other two-hand qwerty keyboard, though.
EDIT
: Were it not for the learning-curve interrupting my daily routine, I would gladly switch over to Dvorak one-handed keyboarding. The day I do this will likely be too late as it is incredible for gaming with the mouse. You can find a link to this style of keyboard(ing) here:
