Ner'Zhul is Gone?

Perhaps I missed something obvious, but when and where did it state for certain that Ner'Zhul had been destroyed and Arthas was now the sole consciousness of the Lich King?

No. Ner'zhul and Arthas fused together into one new being.

That's what I thought, but I remember reading some where earlier (can't locate the link now) that Arthas had somehow dominated Ner'Zhul.

Ner'Zul is probably dominating , but Arthas is stil there(in some way)
If you read the lore about Ner'Zul you see how power he is.
Arthas was a mere paladin while Ner'Zul is master warlock.
He became the frozen throne because he disobeyd the
burning legion . For many years they torturted him until
they bound his soul into some armor . Just read the stuff at
wowwiki :P

Actually, I remember someone posting that they read the new book of Arthas: Rise of the Lich King, title is something like that. They stated that in the book, Arthas kills Ner'Zul. That is why they were 'sleeping' for like 7 years, a fight between the two.

i did not read the book. But there is no way Arthas wins. Hes
powers came from Ner'Zul . Ner'Zul is a very very powerfull warlock , he cant die like that also i would like to remind you
thatt there is a quest in icecrow wich destroys the heart of the lich king and Tirion clearly say " There is no Arthas Menethil left , only the lich king"

Look, blizzard designers made that quest, but if you want to know why Arthas killed Ner'Zul ask the author that book, just passing along the info.

SPOILER ALERT
Just warning you if you haven't read Arthas: Rise of the Lich King.
So, I'm writing this reply on the basis that the new novel by Christine Golden is somewhat of a retcon. Anyways, in the book it is revealed that Arthas was locked in a metaphorical chamber in which he watched himself (represented by a sickly, clearly-faltering but hopeful and resilient child) and Ner'zhul (confident and "winning") fight a battle of wills for grown-up Arthas' soul/mind/heart, etc.
This scene is fleshed out in different areas of the book. In the end...note again, SPOILER ALERT..."grown-up" Arthas stands up, takes the child in his arms and appears to choose good over evil. Then suddenly he impales the child with Frostmourne and Ner'zhul begins to gloat about being unstoppable now that he and Arthas were one. Then, in a shocking turn of events, grown-up Arthas turns and kills Ner'zhul. Arthas then says something to the effect of, "Out with the old, in with the new. Now there is only the Lich King...blah blah."
It appears now that there is no Arthas and there is no Ner'zhul. There is
just
the Lich King.

It's up to belief. There is a lot of confusion and weird things coming out of Blizz right now in concern about the fate of Ner'Zhul and Arthas. The way I view it is, the ending of the book isn't real. It is a metaphor. Arthas (the man) is dreaming as it states in the beginning of the book. He kills himself (the boy) and Ner'Zhul as a way of saying that neither will influence him. That however, does not mean that Ner'Zhul is gone. As is stated, Arthas and Ner'Zhul are now one being. You can't just kill one half of your soul. He's still there, as evident by certain remarks he makes throughout WotLK.
It is a tricky subject, but that is what I believe. Ner'Zhul was a master planner, and was always one step ahead. I have a feeling that in some way, we will see him again, outside of The Lich King (not literally of course).

Basically. It's been said all before that the two souls fused, HOWEVER that doesn't mean that the minds have to. It's very weird, I know. Time will tell I'm sure.

I did play the RTS games, however it has been a while so I don't necessarily remember too much about the personality of Ner'Zhul, or if the games ever actually gave enough info to hint at Ner'Zhul's personality. But from what I've seen, the Lich King we encounter displays a personality very similar to that of Arthas, granted it's a corrupted version.
