Are you recreating these icons from scratch, or are they the exact same files you were using prior to reformatting?

Just to satisfy my curiosity... Try this

That is a known working file from my own Icons folder, that was simply extracted from the WoW MPQs. In other words, if that one doesn't work, something is off with your install. If it does work, then at least BLP images function properly and we'll go from there.

If there's an install problem, try running the blizzard repair utility (called "repair" in your game folder) - it will check all your files and make sure they're not corrupted anywhere...don't do this too often though, it takes a while.

Ya, that doesn't seem right. That is one of the EXACT files I use myself.
Are you sure you're file paths and everything are correct?
In other words:
Path: World of Warcraft/Interface/Icons
File name: some_file_name.tga or some_file_name.blp

That is to be expected from the repair utility. It simply deletes the cache folder because it contains nothing of importance, and is recreated when WoW.exe is launched. It renames your Interface folder in order to back up your addons (as the Interface folder is also recreated if missing when WoW.exe runs).
As for why it's not working, how are you converting the png to tga?
