Good news: You should easily be able to do all the wotlk-launch heroics. ToC will be a little rough, probably and the new heroics should be out of the question for a while.
Bad news: Unless you have a pocket healer friend who will group up with you, you're gonna take a lot of flak for your current health and gear.
Certainly start running the older heroics to start collecting emblems of triumph. In rather short order you can start gearing in T9 and gradually get that health pool up. If you get a better tank ring (you could throw the money at the JC tank ring) and that would put you over uncrittable so then you could definitely tank Naxx, but again you might get hassled cause you're not over geared for it.
Run normal ToC to get more 200 lvl epics and sprinkle in some of the new normals (you've already run FoS I see) while working on the easier heroics to gather triumph badges.

Well... saw that u're DK dw Tank... so... the new patch bring one cool rune enchant for your weapons(you're already using it). I recommend use DPS slow weapons on both main-hand and off-hand, it'll generate more threat, those new Lich King dungeons have some cool dps slow weapons that u can farm all day :P

Dks were meant to tank with a big ol' 2 handed badass weapon. We are only able to DW tank because people were complaining. Without using two Slow DPS weapons your rune strikes will hit like your using your mom's purse as a damn weapon. The point of DW tanking was never to get more health or avoidance. In fact, it falls behind 2-handed tanking for the lower gear levels. At higher levels they may be equal. But raulnor is not entirely correct in one handers being more avoidance and hp.
