HToC is an entirely different beast when it comes to healing for us, IMO. Marked For Death will kill your party pretty damned quick when combined with the massive damage he deals out with that bite of his.
The only thing you can honestly do is yell at everyone to stay close so they can all benefit from the Chain Heals, otherwise with your relatively low stats are going to get eaten alive. As everyone else who is well geared knows, this is a race for the DPS to take him down before you run out of mana.

I had a similar problem the one time I tried healing H ToC. I have slightly worse healing stats than you do, and Phase 3 of the Black Knight was an absolute killer. I barely have to put any effort in to heal Normal, I was shocked how much harder Heroic was. Thankfully the group I was with was fairly understanding, and on our third attempt everyone bunched up close (even though it the group had 2 or maybe even 3 ranged DPS), and I just spammed CH. I thought we were toast when I died, but they managed to burn him down a moment later.

I'm in mostly ilvl 219/226 gear and that last part really messes with me, whereas my resto druid in mostly blues can just pop barkskin + tranquility and get through it just fine.
Make sure you tell your people to pop cooldowns. DK's should hit AMS, rogues should Cloak of Skill, etc. Anything to lower incoming damage

Dude you're fine as far as the stat department goes. I did it with far less stats and without much issue. The main thing you have to do is to tell your ranged DPS to stack on top of you. Once you do that, you should be fine. Just constantly spam Chain Heal and pop Bloodlust at the start. Phase 3 goes fairly quickly as long as you have some decent DPS.
My first time healing Black Knight:
1800 Spell Power (with Earthliving Weapon)
28% Crit
304 MP5 with Water Shield
300ish Haste
You just got to keep everyone close enough to benefit from Chain Heal.

For the third phase of the Black Knight encounter I always use Lesser Healing Wave on the member with the lowest health. My LHW hits around 5k and crits for around 8k which seems to be enough to keep everyone up during this phase.
The thing here is that my shaman have around 490 haste and 2.4kSP. This might be why I can keep everyone up during the third phase but I would suggest you try it and let us know if it worked better for you.

This.
That shadowfrost crap is an absolute nightmare and has 2-shotted me more than once.
Your stats are similar to mine, and I manage it ok. Couple of 'tricks' I use (since 'make everyone bunch up for CH' has been mentioned) :
Before he gets his ghosty butt off the floor for phase 3, pop your fire ele. There's essentially no aggro table for that phase, so don't worry about your ele pulling. He'll provide some decent damage and he's expendable, so it doesn't matter if you don't get heal him.
As soon as your fire ele is popped, don't wait for the black knight to get up, hit Bloodlust before he resses. I find my GCD's are precious for that phase, and quite frankly, you're not going to be fighting long enough to see BL go through its full duration anyway, so it's not going to be a wasted buff.
Only other thing I can think of is to use a haste pot (although not sure if this stacks with BL) and haste buff food.
This fight is really nothing to do with you - as has been said; if your dps aren't putting out the numbers, you're screwed no matter how good you are.

No
this is a race for the DPS to take him down before you run out of health.
Yea, I think I was half-asleep when I wrote that...I meant to say health. If you run out of mana as a Resto Shaman you have more pressing issues to worry about... >.>;;

Isnt everyone suppose to stack on this phase anyway? Not stacking = fail on this phase regardless of what class is healing.
