Nothing is terribly wrong with your setup. You just lack gear.
Id blast through the other heroics as much as you can and get all the ilvl226 badge gear. When you find a well geared group try and get them to take you to htoc. :)
Your spec is fine, really, the only things Id personally change wouldnt do anything for your survivability so not much you can change there.
Certainly would prioritize the
tanking sigil
as your next purchase... replacing a dps sigil with a tanking sigil would help your tanking more than getting away from shield block on the neck piece. ;)
tl;dr
You're on the right track, need more gear.

honestly i tanked Htoc5 with similar gear to yours. the above advice is pretty spot on so i'd listen to what they said try to get your upgrades and farm those emblems. leave Htoc for last unless you get into a good group. you still have alot of emblems to go.
If you get into a decent group for htoc then you'll basically need to learn to survive as a tank. Tanking isn't just about holding agro. it's about holding agro and being able to survive the incoming dmg. whether you have to blow cds, use interrupts, stuns or whatever you can to reduce or stop the dmg long enough to get healed back up. as a frost tank you have 3 cds...IBF, UBA, AMS.
learn to use those 3 as if your life depended on it. Faction champs can be tricky depending on what you get. For the most part when you pull you may want to chain pop your cds at the beginning till 1 champion is down to survive a bit. if not use your army of the dead. sometimes if the group your in isn't very good you'll end up having it up again later anyway. although i'd use it as a last option like you guys wiped twice already and you want to try use it just to get past the faction champions.
paletress can be tricky if your group sucks. Set the paletress as your focus target so you can see when she's doing holy fire or renew. both should be interrupted to make the fight 10x easier. Eadric is easy as stated above.
Before starting Black knight decide how to do the 2nd phase, kill the zombies or burn him down. If you have a group that's trying to burn thru him but is kinda failing your in trouble the zombies will kill your range and there's no way to hold onto them. getting out of the green stuff is a must and moving away from exploding zombies is also a must. Kite the boss a bit to get away from the zombies when you see hunter marks above their heads. During 3rd phase forget about holding agro you should have more then enough. Pop your aotd if it's up, bring up your ghoul, get your diseases on and start deathstriking. If your group doesn't kill him fast your army will absorb some of the aoe going out. Deathstriking and popping AMS will help out the healer a bit so they can spam heals on other people.
Sorry if that was lengthy but good luck :) i did htoc with 25k unbuffed hp so it is possible just gotta do your best.

Are all the healers spoiled with 30-40k+ hp tanks?
I'll give you a healer's perspective on this, and say
yes they are.
We get used to healing tanks with tons of hp or who are less spiky than DKs and we get complacent. Sometimes we go to sleep on the job. That is not a problem with the Tank. Although especially in the case of DKs, it
is
important that they utilize their cooldowns properly.
A few months back I PuGged into a group for H ToC5 and had a DK tank with 30K hp after my Fort buff. One of the DPS actually whispered me to ask "Are you sure you can heal him with his low HP?" I had to resist laughing, as my first successful H ToC5 run had been done with a tank with only 24K hp buffed. I simply said "Yes, provided he's def capped it will be fine" and the run went very smoothly (tank hp as it is
now
I suspect that same dps would have simply kicked the tank).
Fix up your gems and your enchants as suggested, and find a few good pocket healers who don't get caught off guard with a tank who's harder to heal than a 50K hp Druid. You'll do just fine!

If you have a group that's trying to burn thru him but is kinda failing your in trouble the zombies will kill your range and there's no way to hold onto them.
Remeber though, as a DK, you do have an advantage here that no other tank has, which is you can throw DaD over on the healer/ranged and hopefully grab some ghouls off them, all while still sticking with the BK.
not everyone has morbidity and normally when he starts summoning all the ghouls i tend to drop DnD then. It's usually not up when they decide that i'm not worth their time and they'd like to go hit the squishy priest over there. However, usually if i have dps that can't burn the boss i'll just have them aoe the ghouls, they die quick and technically you don't even need to kill them. just dps them to a certain hp and move away they finish themselves off.
Although regardless of my group we've been aoeing them anyway...groups with 4-5k dps kill the ghouls in like 5 sec. it's just fun to aoe and see all the numbers pop up...deathchill/howlingblast ^^
