In a group that knows everyone's rolls, and is just well enough geared for an encounter, it's usually the tank that dies first.
Not because the healer sucks, not because the tank had to panic button to save an overaggroed DPS, but due to the fact that the tank might have been hit by an unlucky succession of attacks and abilities.
If the healer knows what to do, then the tank sould never die or a normal attack, however, say, a warrior's shield block was taken down too quickly, the warrior gets crushed, and the mob does a special attack in the same second, due to a timer, that hits for another 6000, chances are your tank will be dead either way.
If the tank and DPS knows what they're doing, the DPS won't overaggro, if they have a lucky string of crits, the DPS should check the threat meter and lay low for a couple seconds,
the tank should know how to utilize threat generating abilities and taunt/righteous defense/growl, the DPS shouldn't be pulling aggro unless they're overgeared for the encounter, in which case, they should lay low with the DPS at all times anyways.

Depends who I am playing. When I am on my Priest it is usually me if the tank sucks or the DPS if they pull aggro too often. When I am on my Warrior it is usually me, I try to control the fight and keep everything on me but healers can't keep up with that even on trash. Shoot at level 62 I was tanking Ramparts and a level 70 Priest let me die on the dragon boss, I say let me since he wasn't doing much healing our DPS (2 rogues and a hunter) were just burning the boss down for the most part.

Or tanks doesn't understand how to tank and blocking mechanics.
healer: over heal 2 much and pull someone
Which means: Tank and DPS are slacking. Over-healing does not cause threat.

or if the tank is fast they can taunt so the healer doesn't die. I never have that happen when I tank.
The most often times of people dying is when I tank multiple mobs and dps think they can AoE everything. I am a warrior not a paladin.
Tank should always die first if people can control theirselves.

Over-healing does not cause threat?
I've seen many a Priest go down because they throw up a G-Heal right as the Warrior is about to lay down his first Sunder... CRIT... game over, mob goes running towards the Priest.

No. It does not pull threat. You've never seen a priest over-heal and pull threat. What you've seen is a priest heal for a lot and pull threat. But that's not over-healing.
