Dodging/Avoiding and Subt talents that stack

I'm playing combat and have not played a subtlety build before, so bear with me on this question:
It seems that many people do not use Ghostly Strike when going into Subtlety... perhaps b/c they use Hemo instead. Anyway, I'd like to know if anyone has used it in conjunction with Setup and Enveloping Shadows. It seems that if you were to take some key talents such as: Lightning Reflexes, Evasion, Ghostly Strike, Enveloping Shadows (and perhaps Sleight of Hand and Heightened Senses and Cheat Death) that you would have some serious combo point generation.
But do all of these count as 'dodging' an attack, which is what Setup calls for? The wording of "avoiding" might not equate to dodging, which is basically my question. Do all of these stack with Setup?
