To answer the question: Yes; if you don't have premium, no special icon. And if you don't want to pay for a year, don't; just get a couple month sub or the like.

Is there a price difference because of different countries or something? I see the yearly price as $29.99.
Regardless, the price is for all features... and to my knowledge there isn't a way to only give you one of the features for a lesser price while allowing the others to go suspended.
May I ask why the custom icon is so important to you?

When looking into Premium too, Premium is Zam wide.

The Zam network is always growing as well. The biggest example of this is Lolking.net. Already the site is adding new features, and with the future looking great for League of legends the site is only going to keep growing.
If you're not a multi gaming person, and you just love WoW and Wowhead, it's another great reason to be a premium member. It gives a little something back to the site. We love to see the premium member community so involved in the forums. I joined Wowhead a little over 3 years ago, and before I became a moderator I was a premium member for half that time. Once I got involved more in the forums and using more and more of the database it was almost a logical step to associate myself more with the site and feel more at home.
Just my 2 cents on the issue. ^_^
