Debuffs and tank swapping

This might be a stupid question, but I prefer to ask them here instead of in a potential raid situation.
How do you handle the tank swapping on a certain amount of stacks of debuffs? Do you make a whispermacro to whisper the other tank to tank off of you? Or do you expect the other tank to be monitoring your debuffs somehow (raidframes, special addons)? Or is this just decided between the tanks when the raid is put together?
(I'm talking about pug raids as guild raids often have vent, ts or similar for such matters)

There's about a million ways to easily monitor it so I would never expect to have to announce it to the other tank unless something special was going on.
When it's a boss stacking whatnot the OT will often be attacking the boss too and should then be able to spot debuffs via target-of-target.

Firstly, i like the method of taunting the boss as soon as my debuff wears off, without the other tank having to tell me. I could not taunt with the stacks still up, and when they fall off, why not getting it right then? The other tank can do exactly the same, and there is no need to interrupt the new episode of "1000 and 1 stories of how Brudus was drunk" which gets aired (by Brudus) on Vent.
Secondly, i use the "expanded focus" option, and set the other tank as focus. In situations where you do not "wait for your stacks to fall off", i monitor the offtanks debuffs and taunt when needed. I use this to see when the other tank has a blood rune from Saurfang, or to check his and mine ammount of "Putricide AoE thingie" stacks.
DBM tells you how much stacks of X or Y the Z guy has, but that can get lost in floods of other warnings. (starting with "Oh sir i suggest you move out of the nasty thing you stand in" and ending with "You just cleared the Xth level of BEJEWELED GZ")

A lot of how tank swaps happen depends on the fight and who needs to know.
Deathwhisper; I just taunt when his debuffs get high. Sometimes I'll call in vent that I'm taunting, sometimes not.
Saurfang; I monitor his debuffs (and DBM warning) and just taunt when he gets Rune. I never say anything.
Festergut; I monitor his debuffs, I call on Vent when he hits 8 stacks that I'll be taunting in 5 seconds, and then call on Vent again when he hits 9 stacks and I actually taunt. This is more for the Healer's benefit than anything else.
Putricide; Phase 3, we pre-call all taunts in vent so healers know to switch. I hold him until 2, then DK OT taunts (calling in Vent) and holds it to 2, then Paladin calls his taunt in vent and taunts and holds until 3, then I call my taunt, blow CDs, and he dies.
Sindagosa; Phase 3, there's a lot of vent chatter with directions being called and all of the tank swapping is coordinated there as well.
For something like the new Ice boss in VoA, I just taunt when my stacks drop.
What it comes down to, really, is whether the tank swap is going to be a burden on Healers, and if the healers can be expected to know that a swap is about to happen. If there's a real threat of the tank dying, the taunt should be announced in vent (or made in a /RW I guess).

With 1 tank its easy. I just put him on focus and make sure i can see his debuffs in the UI. I just taunt whenever i feel its the right time.
With more tanks its harder, i got macro which says:
/s S W I T C H
I am just spamming it whenever i want the switch. Other ppl have similar macros.
Or we just say on vent if its a guild run (not doing many pugs).
Similar as in Porcells post.
