I don't have any issues using the method I described to add new icons... Try naming it something completely weird like "custom_icon_1" or something, and then fixing the icons on the macros that used it, and see if that fixes the drag and drop issue.

It sounds like either your not saving the file properly (24 bit uncompressed TGA w/ Alpha channel @ 8 bits per pixel) or somewhere along the line your file is getting corrupted somehow. Sorry I can't be of much help, but I'm not exactly sure what you're doing wrong.

Id also like to alter some of the in game icon art...Ive downloaded and installed the "Addon Kit.exe" into the WoW directory...when I run it - I get "Install Interface data" and "Install Interface Art" - but not "Extract Interface Art" (like was suggested)...I tried the "Interface Art" option and got a folder named "Blizard Interface Art", but the macro folder in it contained only .tmp files (and not .blp files that I could open with XnView)...
Sorry, it's been a while since I've actually used it :P Install interface art is what you wanted. You want to look in the Icons folder of that folder to find all the icons (ie. world of warcraft\blizzard interface art\icons\) you want to save YOUR icon to world of warcraft\interface\icons\
