You can run the exe "Addon Kit.exe" inside of your wow directory, and then select "Extract Interface Art" This will create a folder named "Blizzard Interface Art" in your world of warcraft directory. Inside is all of the art files for the UI, including all of the item/ability icons.
From there, you can hunt down the icon you want to change, copy it, convert it back to a TGA (they're all BLP files), edit it, and then save it into your Icons folder.

Oh sorry, that was my fault. I completely forgot it isn't there by default... I grabbed it like 3 years ago and stuck it in there so I wouldn't forget about it.

For your first problem, try placing it in your Icons folder named EXACTLY how it was in the interface art folder (you may also need to convert it back into a blp). They removed a lot of the MPQ overwriting ability that people had due to it being exploitable, but I cannot recall if they removed the ability to overwrite interface art or not.
As for your second issue, I have no clue what could be going on. I've never even heard of someone having that kind of issue before :/ Sorry I can't be more help there >_<

Now that I think about it some more, I'm not sure if you can go back with xnview, not that it
should
matter. A BLP file is just a TGA file with different header information. Try leaving it as a TGA, but completely restarting wow. Some things require a full client restart to work (like toc file updates, or if memory serves, MPQ overwriting) because they're only checked at client start.
