Biggest Victory: Finished off Sartharion w/3 Drakes after the rest of the raid wiped as a healer. Took him down with a tick of Flame Shock, and went down right after he did thanks to an incoming lava wave. XD
Biggest Screw-Up: The inevitable "Release Spirit instead of Soulstone/Reincarnate" during a boss fight when I was MT Healer.
Dishonorable Mention: Not my own, but I'll share anyways. Our Prot Pally tank literally ran off of the edge against Kologarn because he didn't realize there was a drop-off in front of him. We all thought he had been one-shot, but after that realization...needless to say we needed a 10-minute break to catch our breath from laughing.

Biggest victory: Undying, having been denied the previous week by a glitch on KT where 3 seperate heals failed to land on someone who had been

Biggest screw up: Every time I ever totem-pulled, missed a Bloodlust opportunity, self-rez'd immediately before a boss AoE, let the tank get out of healing range, or cast lava burst without a flame shock.
Edit: I'm glad you added the qualifier "any class", because the Undying victory was on my druid (although the fail the previous week was the shaman). :)

Biggest Victory: Fighting in pvp zone in Nagrand an 80 lock hits me (66 at the time) with curse of agony in the air, he falls to his death. I dismount, remove curse and slow fall. I fought an 80 lock with a 66 mage, they died I didn't.
Biggest Failure: Flying around Sporegarr I saw an herb I wanted to pick, I dismount, I forgot to slow fall or blink. I died from my own stupidity.

Bigegst Failure: Going through nearly all of Naxx with no weapon imbues on.... figuered out when someone finally said, "Why are you so low on DPS?"
You can configure Power Auras to show a symbol on the screen when your imbues fade, just a little tip, its a very useful addon, can also use it for procs such as 5x maelstrom wep etc :D
Biggest victory: Probably on my moonkin, when I did a little Panzerkin for a while :D. Or on my priest when me and 2 guildies (mage and warrior) kept an entire horde raid out of WG when we hadn't control. Kept wiping them on their way into VoA for about 30 mins when they finally thought about coming out and killing us together. Was so funny that in the end we didn't even fight back, it took 10 horde to kill 3 ally XD.
Biggest loss: Probably when doing some random skirmishes on my disc priest, just after he hit 80, me and a feral druid guildy were bored so decided to pvp for lols. So, started losing skirmishes quickly, one round we came up against a warrior and resto druid. Turns out the warrior was in my guild too, this guy......there aren't words to describe him in pvp, its just insane. Soon as I saw him rape the feral I left, damned if I was gonna let him kill me. Cowardly? Yes. Clever? Kinda..... :D

Not really my screw up but a huge one still:
ToC 5man pug. I joined as DPS, we had a second Shaman as healer. All went ok-ish, the tanks HP fluctuated wildly in the first two fights but we managed without dying. DPS was fine too, me on top with 4k, a Hunter with almost 3k and a Paladin with 2.5k.
Black Knight, the healer likes standing in ghoul explosion and barely survives but then of course to the knights first AoE tick. I try to emergency heal in elemental spec but it isn't enough, we wipe.
Next try I pay closer attention to buffs and stuff since we wiped. The Paladin buffs wisdom on Shamans. I ask for kings, nothing happens. Tank asks "rdy?" and I say "no, waiting for kings". Paladin ignores it. I tell him about mana spring totem, drop it for him to see. Maybe he doesn't know that wisdom is redundant. He still ignores me. I tell him that the 10% extra hp would probably have made the difference in the last fight since then our healer would have survived that first tick.
His reaction: "I'm not buffing that guy at all, he is whispering me insults!". I was like WTF? The guy is a complete moron but I certainly did not whisper him anything.
The tank just runs in, we are still unbuffed ... the healer again likes standing in ghoul explosions, again the healer barely survives and gets insta killed by the knights AoE. Again kings on Shamans would probably have saved us.
Then the tank just leaves the group. I was kind of glad he did.

Biggest victory - Thunderstorming nearly an entire alliance raid on thunder bluff off the elevator.
Screw up - Trying to heal ToGC in my elemental spec. Oops.
