Now I admit I have never healed it before - generally tank it, but some combo's of the three bosses will be harder than others. This does seem to be the hardest fight to heal.
The tank having good gear, does not mean he/she has a clue of when to use their mitigation talents.
Having wiped in groups there a bit myself,
- did the tank shieldwall at the start to absorb the initial brunt of damage.
- did the DPS blow some cooldowns to take down the first target as fast as possible.
- they are stunable, did the tank/dps stun? this most certainly reduces incoming damage, especially with all three up.
- Did any hybid DPS, throw some heals in to help (often they ignore they can heal).
- did you use Lay on Hands when the end was nigh.
Once you have one down, the critical point is past and it seems the second two are somewhat easy.
Of curious note - druids tanks seem to have the most trouble tanking (absorbing) the initial brunt of damage, while warriors and palladins have a much easier time.

Your gear is definitely not the problem. I've successfully healed ToC HC in average ilvl 190 gear before.
What do you mean "spam HS"? Surely something is getting lost in the translation there :)
The combo of the three mobs matters a lot. So far, the most trouble I've had was with the Shaman, Warrior and Mage. The Warrior and Mage both do tons of damage, and the Warrior does an AOE (whirlwind) that not all people are smart enough (amazingly) to get out of... usually blaming the healer when the 5000 DPS AOE kills them xD
I would bet that most of the trouble there was with the group... Not staying out of the purple stuff (sort to speak) or pulling caster aggro. Maybe the tank not knowing when/how to properly "tank" the brunt of the damage he's taking.
ToC HC is, arguably, hardest on Pally healers, but shouldn't be to much trouble if the group doesn't have to many 10 year olds in it :)
... Until you get the phase #3 of the Dark Knight, that is... then most of it is luck xD
Edit: What's with the tank Libram? You should try
Libram of Renewal
or
Libram of the Resolute
to help with the healing.

I wouldnt beacon the tank tough. Hes your primary so i would beacon a melee dps.
That makes no sense...
You never really heal the tank itself. You beacon him and heal everybody else. (Aiming to top the tank off, obviously.)
Keep the changes in 3.2 in mind. Overhealing is no longer ignored by the beacon.

isnt it arcane dmg in phase three? omg im such a noob if it is shadow...
They call it
Shadowfrost
damage.
From the details here on wowhead, it looks like it is classified as Frost damage. Sounds to me like a combo, somewhat like a mage's
Frostfire Bolt
.
(Note that Frostfire is only classified as Fire here on wowhead.)
Not sure if either Shadow or Frost Aura will do much to help, although as the damage in that phase is mostly magical there is no harm in picking one and switching to it.
Biggest problem in part three if I have a dodgy (or, rather; not) tank is that I'm to low on mana to spam the HL required to keep everybody up.
I tend to just pick the lowest DPS and prioritize heals on the others, popping Divine Shield to keep myself alive as long as possible, and try to keep LoH available for when I am getting to low on HP or mana.
Edit: Added info.

Noone ever has to be hit by ghouls exploding.. It takes ages for them to explode when they start casting, and they stop. Just kite the boss around in a circle, and your melee dpsrs will hopefully follow close behind, and wont get hit by a single ghoul.

1st fight: Depends on combo.
kill priority should be:
warrior, rogue, shamman, mage, hunter
note: you must be interrupting the shamman's heals.
If you cannot interrupt, then the kill order is:
Shamman, warrior, rogue, mage, hunter.
2nd: Guy is cake, turn around and sleep your way to lewt.
Chick causes more trouble.... not to bad though, just keep beacon/SS on tank and flash your way to loot, shocking and HL when necessary. (dont forget your mana cd's)
3rd: Pop Divine Plea between phases... be sure to cancel before the phase starts.
P1: Easy, go light on mana usage. Cleanse, cleanse, cleanse.
P2: Nuke him down. But dont use cd's, they are for phase 3.
Keep beacon/ss up on tank(and the hot if you can) Have dps run out of range as the ghouls are dying/exploding. (aoe helps but is unnecessary as they all explode after knight is down... just have dps/tank run out of range when they start to grow.)
Dont forget to judge light on the boss at start of each phase for the healing and for the haste. (1 judge should be enough for each phase unless your dps is fail)
P3: Prior to his showing up, if you are low on mana, pop LoH on yourself. refresh Beacon and SS on tank.
Pop DIvine illumination and spam HL on a rotation of yourself and all the dps.
This is all about the dps. After a little while he will 1 shot everyone.(damage output increases steadily until damage is greater than the standard health pool) All you need to do is keep the dps from dying before they are 1 shot.
HL spam will do that. FoL will not cut it.
Not even close.
Save your 1 mana pot for phase 3 if you need it.

Having healed & tanked the place I will say it ranges from a major PITA to a cake walk depending on your luck & group. To me it sounds like you had bad luck & poor DPS, even with the harder combos having good DPS is a life saver. The fight shouldn't have lasted long enough for you to go oom, unless they were too dumb to watch where they're standing. You can't save people from their own stupidity.
