If i'm ever being asked to MD something like adds on the fly mid-fight, i have a simple macro with the add tank's name already in it so that i don't even have to target them. I usually have both that macro and my regular MD macro on a bar.
It's literally just:
#showtooltip Misdirection
/target <tank's name>
/cast Misdirection
It definitely isn't fancy, but i've found it to be fool-proof.

It does, but i don't really care in this situation for a couple reasons...
1) I have to switch targets to the add anyway
2) I don't want to accidently MD the boss instead of the add if i don't switch before hitting my macro
And yeah, im not particularly great with macros yet. I could tell you any macro you could possible imagine for FFXI, but I'm a pretty late WoW convert.

My solution is to have another macro in my macro list I can just drag down there.
Not having to swap out macros when you're going from farming to raiding is nice, I'll put up with the annoyance if I'm on one of the small number of cases where I can't bother to focus on the MT/OT.
