Trollwoven Spaulders need to go, replace them with Spaulders of the Giant Lords from the Sons of Hodir (need to be revered, if you don't have that yet, get some Savage Saronite shoulders to tide you over). Gored Hide Legguards should be replaced by Staggering Legplates from H UK or Legplates of Bloody Reprisal from the Wyrmcrest Accord (need exalted). Buy or get a JC to make you a Titanium Impact ring to replace Shadowmight; Hemmoraging Circle from H Gun would be an upgrade as well. Get the Flame-Bathed Steel Girdle from H Nex or the Barbarian Cord (can't remember the exact name) from the vendor in Dalaran, costs 40 emblems of heroism. Replace Argent Skeleton Crusher with either the colossal skull cleaver from H HoL or the 2h sword from the Argent Tournament. Run H HoS for a bracer upgrade, or if you have the money (usually ranges from 1-2k g), buy the valor bracers from someone ("Sentinel" something). Save up emblems (or hope for a lucky raid drop) to get a better chestpiece. And while its a minor upgrade, I'd also get the Blood Inured Sabatons from the Ebon Blade (exalted) to replace the Spiked Titansteel Treads.
Gems and enchants: strength is king, followed by crit. You have a lot invested in hit rating, and while you kind of need it with your current gear, once you've acquired the items listed above, it won't be an issue anymore. So bold scarlet rubies and smooth autumn's glows are what you want. As for enchants, get ap wherever possible (bracers, gloves, boots), agility for your cloak, and super stats for your chest (don't waste money on powerful stats until you get a chestpiece worth the cost).
You do all that, and your ap will be around 4k, hit rating over 300, crit over 30%, and expertise in the teens.
AP on boots? Bad idea. Icewalker al the way.
