If all of these are on the GCD then you cannot activate them in a single push. When you use something on the GCD it stops anything else from being used until the GCD time frame is up.
You can kind of get around this with a cast sequence macro.
#show
/castsequence reset=15 Soul Burn, Swap Soul, 15, Dark Soul
This macro will cast each spell in the order you put them in with one key press at a time. The problem with using them this way is that a cast sequence macro will cycle through even if you cannot cast the next spell. So you will have to press this button 4 times, waiting for the GCD to cycle before pressing the key again.
Also you can modify the reset condition in there, 'reset=15' means if you press the button once then it will wait 15 seconds before resetting the macro.

Ok Did a little research after posting this. First your trinket is either 13 (top) or 14 (bottom), 15 is your cloak slot, so that may be one problem. Only one of the spells there are on the GCD, Soul Swap. So I can suggest a better macro that does what you want as follows:
#show Soul Swap
/cast Soul Burn
/cast Dark Soul
/use 13
/cast Soul Swap
When you make a macro with multiple spells in it, only one can be on the GCD and it must come last in the cast sequence.

Jurasco's second macro should work fine. Neither Soul Burn or Dark Soul invoke the GCD

Remember you also need to not have any spells "qued" when you press the macro.
Pressing the Macro while casting something else can also result in you queing the SoulSwap without using the cooldowns.

Don't use cast sequences for your rotation. They murder your dps since they prevent you from queuing up another spell until the current one is completely done. Ideally you'll be mashing your filler spell between casts of DoTs so your next spell gets cast ASAP. And general rule of thumb is ABC, Always Be Casting. Anytime you're not casting is a dps loss.
Icy veins
has some decent guides for rotations based on spec. Practice a bit on dummies. Once you're fairly comfortable do some dungeons.
