Throne of Tides bug?

I looked in the archives and couldn't find what I'm looking for. I was doing the normal (5 man)Throne of Tides yesterday and several odd things happened. First no one in the group could get the quest. The ! mark showed up on the dungeon map but not on the playing screen. You could talk to the quest giver but there was no ! over his head, so no quest. Another thing, I died on the last boss Ozumat. As I tried to reenter the instance to recover my corpse, it wouldn't let me. Got this message 'You can not enter instance because an encounter is going on'. I tried repeatedly but kept getting same message. Finally after the message 'Throne of Tides completed' came up it let me reenter the instance but I didn't get credit for the dungeon. I tried to run back through to see if I could get credit but instance had reset. It was not time for the daily reset. Was this a bug? and if so should I open a ticket?
Thanks

About the whole re-entering the instance thing, you can't re-enter an instance while your party is still fighting the boss, so that's why you could only enter once the boss was defeated.
I'm not sure about the rest of the problems though. Sorry.

It sounds like your party engaged the boss and killed him while you were fetching your body. Because they had already started the fight, you couldn't get back into the instance, and since they killed him while you weren't inside, you didn't get credit. This isn't a bug, this is just your party being distasteful and not waiting.

SquireKel,
Does it matter that I had started the last boss fight too but died before he was killed?
Thanks

No. If you die during a boss fight, don't release. You won't be able to get back in (except in a few select cases) and won't receive credit for anything killed while you're outside the instance.

I don't see how this is distasteful of the party, the only thing they could've done to prevent this scenario is wiping, and you can't expect a random LFD group to do that. In all honesty it's the fault of the person that releases, as you should never release during a boss encounter unless it's absolutely clear that it's a guaranteed wipe anyway.
@leonidas: Unfortunately it's not possible to enter a dungeon while a boss encounter is in progress, that is why you should never release your corpse during boss encounters unless it's a guaranteed wipe anyway.
The case of the dungeon resetting most likely happened because all your other party members had left the group, which causes the instance to reset similar to how it would if you manually clicked "Reset all instances".

If you'll note, when I made that comment it appeared that the OP was dead prior to the boss pull. I do stand by that statement - if a party pulls the boss when a party member is dead, I think it is distasteful.
However, the OP later on made it clear that s/he died during the fight, in which case it was his/her fault for releasing during the fight.

He did state he died during the encounter in his original post, but that aside, I agree that starting a boss encounter while not all members are inside the instance is pretty despicable.

There is a long quest line to be completed before being eligible for the Throne of Tides quests however the ! will still show up on your minimap.
When you die fighting a boss never release your spirit, stay put and wait for a battle res or for the boss to die.

Another thing, I died on the last boss Ozumat.
The way I read that is that s/he died during an encounter with Ozumat, but because most people know not to release in a boss fight by the time they hit the 80s, I assumed s/he meant the party wiped, and then re-pulled the boss while s/he was still running back into the instance.

Fair enough.

However, it also sounds like the group disintegrated as soon as the boss was killed, and before the OP got back into the dungeon, so they entered a fresh instance with all trash still living (that, or it had respawned, if the run of the dungeon took a while)
