Good god that's a long guide. I'm bored enough that I might read it, will edit with comments maybe.
Edit: Through the first post so far. Are you by chance female? The excessive tunneling on character appearance just seems really out of place with the rest of the guide. I always show helms/cloaks/etc. regardless of the graphic, several of my characters would still have the starting shirt if my old guild didn't have a joke policy for shirts, and in general I couldn't care less about what my character looks like. Maybe I'm just the exception to your rule, but as far as I know, very very few of the truly good players that I know care about what they look like in game.
Edit 2: You called Rotface Festergut someplace in the first post, but that was the only technical error. I don't necessarily agree with all the archetypes you laid out, but it was a good read none the less.

I could have created 20 successful PuGs in shorter time than it would take me to read that ridiculous wall of text.
It's not a wall of text because it is properly formatted.
That said, it is huuuuuuuuuuuuuuuuuuuuuuuuuuuuuuuuuuuge. I recommend what someone else said about breaking it up into a post per section. I'd gladly delete my post in order to not break up the guide.
There's nothing wrong with a DPS Warrior buffing Commanding Shout if you have a Paladin for Might and no other Warrior.
And what is a "M.A.W."? A Medium Anti-tank Weapon?

I'm sorry. I like your post but those "avoid these people" stereotypes are utter nonsense and seem largely inflated with superfluous text to sound accurate. What's wrong with someone who has no raid experience prior to ICC? I came to WoW late and nobody does anything else, I got lucky and got into a guild starting on ICC.
The advice is sound but I think you spent way too much time describing the "people to avoid" and arguing trivial things at that.
Actually, reading more of this, this has got to be a joke. Someone in ToC-5 and ICC 5-man gear is bad over someone with T7/8 mix? Someone wearing an "ugly cloak when their race has a nice ass" is an issue? Someone using a mix of gear without a thought to visual cohesion can be a problem? Give me a freaking break.
This "guide" is nonsense, and any good point is lost in a sea of idiotic guidelines that mean nothing.

Sorry friend, but it still sounds like you're picking at hairs to draw a line between "good" Puggers and "bad" Puggers. Visual cohesion isn't worth anything, especially with better items sometimes don't match visually but are an improvement over items that look better. It's a nonsensical argument IMO. Same like your "player stereotypes". Someone who is "fat and ugly" is a bad player? Someone who is "white trash" as it were (the "degenerate") is bad just because of that? I'm unemployed and live at home again, does that mark a strike against me being a good player,

Like I said, I applaud your guide but I think you're trying too hard to draw a line between good and bad players with things that most people aren't going to care about, and trying to make trivial things sound like important considerations and potential "red flags". It sounds like an employer trying to justify only a specific classification of worker by using silly things to disqualify otherwise-qualified applicants.
This might not be your intention, and I certainly can appreciate the obvious hard work and thought you've put into the guide, and I thank you for your work in posting it as I found it a good read, but a lot of it comes off as being elitist and/or outright trolling trying to make a mountain out of a molehill under the guise of offering advice for a successful PuG.

Great guide. After reading it, I noticed that a lot of players are really a mix between a few of the stereotypical players you listed.
On my warrior, I love to do PvP, it's what I do PvE for. But I'm also on and off of the computer a lot, and enjoy some casual things. I tried getting into a raid guild, they're not very good, but I try to run with them anyway. I'm also a nerd in real life, very to my self, and quiet. I'm very assertive and sometimes even aggressive to the 'morons' in pugs. So, I guess I'm what you call a 'Super Casual PvP loser'. That's a good one.
Personally, I've never lead a pug because I'm insecure, and unsure if I'll lead effectively or poorly. Though, I should probably try. Thanks for the good read.

The part in which you mention which types of people NOT to invite....Are there any other types of people?Basicaly you must invite someone that doesn't talk, that follows tactics,Doesn't roll on everything that his class can use , that doesn't go AFK and doesn't use gearscore.
People like that do exist...but they join guilds. You should use the people you find and do the best you can with them.
Also...doesn't wear matching gear? Huh? Big whoop.
