I generally stagger my haste CD's during a fight. Fights generally last longer than 5 mins so you have ample opportunity to use them multiple times.
I open up with a MD to the first tank and use my rapid fire then. Somewhere during the fight someone pops BL so that's the 2nd CD. If the fight is long enough, my rapid fire CD is up and I use it again. As the boss gets closer to Kill Shot range, I save my Readiness for that. I generally spam all my CDs including KS, then use my readiness and KS again. Pop Rapid Fire when the other effects are off.
The reason I stagger them is just incase something happens. If you blow all your CDs and you get hit with a boss ability or something, you've just wasted it. Also the Engineering Haste enchant doesn't stack with other haste CD's as I've been told. I use the rocket gloves engineering thing myself.

but i don't understand your reason to not stagger your cooldowns, because in most icc fights you can time your cooldowns with bossmods so you don't get hit by some boss abilities.
Not everything is predictable. LDW's MC, Fester's Vile Gas, Rot's slime, PP's slimes targetting you, etc. There are a lot of boss effects that target random people and if you happen to blow ALL your CD's within the time you get hit by something like that you lose it all. Imagine if you just pop'ed everything and got the slime on Rot, and had to kite it over to the slime tank. It would have all been used for nothing.
In terms of max'ing dps, I'll have to let the others answer this one. To me its all additive, and accumulates your dps done during the run. If you poped them together or separate, it should lead to the same overall dps. If you stagger them like I suggested, there is a better chance you use them to their max uptime.
