I seem to be taking too much damage...Any tips?

Okay so I've been tanking on my warrior for a while now. I've had several people tell me to just roll a pally, that warriors are useless right now, but I decided come hell or highwater I was going to warrior tank.
This is my Character:

I main tank everything for my guild. ICC, ToC, Ony, etc. Some nights it goes well, but other nights it feels like I might as well be wearing cloth.
We're up to Saurfang in ICC, but on nights like tonight Marrowgar melts my face.
I try to manage my cooldowns. I use SB whenever it's up if I'm going to be melee'd. I use Last Stand whenever I see red, and SW whenever my healer is in trouble.
Even on my good nights I just seem to take way more damage then my OT counterparts, when say I get taken down and they have to solo it.
tl;dr: What can I do with my armor, gems, trinkets and spec to "tank" better?

My biggest advice is to get rid of those two trash trinkets. Avoidance trinkets are useful in a very very very small circumstance. Honestly I never equip mine anymore. Get a Glyph of Indom from Triumph Badges and a Black Heart at least.
You are quite a bit over the defense minimum, so you can put Armor on your cloak and 275 Health on your chest.
Other than that, there's nothing you can do besides get better gear. Your gemming is perfectly fine.
So two trinkets and two enchant changes. That's all I can offer as far as immediate fixes.

I don't know hit rating is as important to tanks compare to dps but yours is low. I know parry gibbing is now removed so expertise should be less important but with hit rating not capped, you attacks should miss from time to time.
Expertise >>> Hit. His expertise isn't capped either (nor even soft capped) and having the boss dodge or parry isn't exactly better than having the attacks miss.

I'd replace the defense/stam gems too as u should never gem for defense unless u need to to be capped and you dont.
Your gems should pretty much be 30 stam down the line except one red for your meta.
There's nothing wrong with using Defense/Stamina gems to pick up stamina socket bonuses. It's a personal choice to pick up +6 stamina bonuses, but you should definitely pick up +9 stamina bonuses.
For his particular gear, he as a yellow+12 in the helm which he rightly put a Defense/Stamina gem in. His shoulders are a red+6 which is fine to pick up, chest is a yellow+9 which he has right, legs are a yellow+9 which is right, and his gloves are a yellow+6.
So of all your gems, the only ones you should possibly change are the Shoulders and Gloves to be +30 Stamina instead of +10 Defense/+15 Stamina, but that's a just a choice for you. There's nothing wrong with going for a total of +10Defense/+21 Stamina as opposed to +30 Stamina.

You're defense is pretty high, you are at 574 defense at the moment. You're avoidance is pretty high as well, and I'm assuming that is what you are gearing/geming/enchanting for. You're melee stats are pretty decent as well.
I would personally suggest you switch and gear/gem/chant for Effective health. There's a thread above in the stickies somewhere I think that should help you. Basically you would boost your health to much higher levels so that you take more damage, but it's easier for your healers to heal you. T
he tanks in my guild have around 42k~45k health unbuffed. Fully raid buffed they are sitting at or close to 50k. They get hit, but since they have such a high health "reservoir", it's easier for the healers to predict that damage and heal accordingly.
Replace your trinkets with stam trinkets. Regem for mainly blue stam epic gems. Ironically you have a +18 stam enchant on your shield vs defense, switch those around. Aim for at least 38k health unbuffed and you should alleviate your healer's heart attacks.

There's nothing wrong with the +18 stamina enchant on your shield. That's one of those personal preference enchants. It's still a pretty good enchant.
There's is nothing wrong with +18 stam on your shield. My point was that the OP was gearing/gemming/enchanting for a high defense value (574) but didn't use the +20 defense on shield for some reason. Just didn't make sense to me to go full defense and leave out one of the main enchants to add even more defense to your total.

I wasn't gemming for Defense particularly. I was gemming for my yellow slots, which much to my chagrin I have an abundance of. In my mind +26 Stam +10 defense for slight avoidance upgrade is better than +30 Stam. I'm taking straight Stam wherever I can, but after reveiwing my gems putting +30 Stam in all slots is worth about 250-ish health. If I could have gemmed for dodge or parry and stam instead I would have, or just plain old straight stam.
