I would generally not recommend using all your cooldowns at the same time. It's typically better to use eg. shield wall to keep you alive during its duration, and then if necessary pop last stand when shield wall is about to run out. Enraged regeneration is so weak it doesn't really matter, but you might as well use it with last stand since it'll do a bit more healing then.

I have Shield Wall on an annouce macro alone, then I have Last Stand macroed with both trinkets. On something like late-phase Gormok or 3-stack Festergut or hardmode Iron Council, it helps to chain together One, then the Other to lengthen the amount of time you can survive.
Besides, making two separate macros doesn't preclude one from hitting both at the same time if the situation warrants. There are just many times where just one will do the job just fine.

I have Shield Block bound to Shield Slam. This keeps shield block on CD, for bonus mitigation and damage on Shield Slam. It's really simple, just:
#showtooltip Shield Slam
/cast Shield Block
/cast Shield Slam
I've been using the same one all through WotLK.
However, I've been considering a change to it, because in certain situations you still want to not use your shield block right when it comes off cooldown: For example if you'd like it for the mitigation when the boss starts hitting harder/faster or whatever. So I'd actually like to ask if anyone knows how you make it, by default, work like the above, but with the use of a modifier key (ctrl, shift, alt) skips shield block and just shield slams?

Seconded. I never, ever have Shield Block macro'd. I always use it at the beginning of trash pulls. First pull, then position, then, Shockwave + Thunderclap + Cleave. Once the Shockwave stun wears off, Shield Block goes up.
Shield Block can make a hell of a difference when chain pulls happen, and after using it in the initial pull, I won't use it again for just such an occasion. Besides, having it go off at the end of a trash pull is just plain annoying, as it's completely wasted.

30% max HP? That's quite a bit for a tank -- not to mention that with Last Stand, the healing
is
increased. It can make quite a difference when combined. I've used it to make it through a boss fight after the healer goes down, along with solo'ing group quests.
However, I do agree you shouldn't be popping all of your cooldowns at the same time. It's a lot of overkill. The only time I've done so has been while leveling and grinding 80 instances (still gearing my Warrior for heroics) to make up for so-so healers in boss fights.

30% of a tank's hp is not much compared to the constant high amount of incoming damage in a raid. In fact, with the spam healing that is traditional, in most cases it's negligible.
For soloing it is much more useful; for 5-man content it's decent depending on the circumstances.

Just out of my head the easiest way to do so would be:
#showtooltip shield slam
/cast shield block
/cast shield slam
/cast shield slam
That should work. If you want to make absolutely sure it skips the shield block i believe you can use /stopmacro command like this:
#showtooltip shield slam
/cast shield block
/cast shield slam
/stopmacro
/cast shield slam

one thing not mentioned here - i created a macro for each of my main abilities (SS/rev/dev) and added /startattack at the beginning. that way, even if i don't have any rage at all, at least i'll start attacking.
example (using ? icon of course):
#showtooltip
/startattack
/cast Devastate
i also macro shield block with shield slam; if i think i'm going to need to use SB manually (last time i remember needing it was thorim HM), then i just edit the macro to remove SB before the fight starts.
last stand + regen + trinkets is a nice combo, and i also keep shield wall separate.

Thanks. Was trying to use that nomod thing but couldn't get it to work so I kinda gave up.
@marklartank: Yeah, startattack macros are nice. I don't have them for all my abilities but at least one or two is good.
Wish I could share some useful macros, but I don't really use more than that. Used to have intervene and berserker rage macros and all that, but they've "nerfed" that :P

Does anyone know a macro that would cast Intervene if i target a friend, and would cast Charge if i target an enemy? (also if it could cast Intercept on the enemy anytime Charge is on cooldown, that would be neat).

This is my all in one Warbringer macro you seem to be talking about:
#showtooltip charge
/castsequence reset=15 Charge, Intercept; Intervene
It has two flaws.
Sometimes if you double click it Intercept goes on cd as well, even though it wasnt used.
Intervene has build in ability to Intervene the targets target, which means you can possibly target opponent targetting your raid member and still be able to Intervene that party member by clicking Intervene.

Zakkhar, those macros of yours don't seem to work. Haven't had the time to test until now. They both work fine when not holding shift (casting both shield slam and shield block, when available), but when holding shift they both do nothing - no shield slam, no shield block.
Tried some other setups that got the same behavior.

Hmm, wierd, i tested them before posting and they worked fine.
I even redesigned one of my Arms macros today to work similarly and it did work.
You sure you dont have any keybinding conflicts?
I did have so i had to use left click + shift.

Yeah after some substantial fiddling I found out where my keybinding issue was. Turns out both your macros and the ones I had tried were all fine, but my keybindings not so much.
